Form for a Complex Paid Pond Club Web Site

The complex paid pond club pond site costs $20. That is a one time fee. You can alter or update the site within the boundaries of the plan as often as you need within reason.

In order to fill out a form for a pond club, you should be an officer in the club or someone important to the club. If you are a member, please have an officer fill out the page so it is official.

This first section is information that I need to set up your page. Except for the subdirectory name and category of your site, this information will NOT be seen by others or shared with others. You must fill in all portions of this section.

The forms I am using only allow for one attachment per form (found that out after I paid!).

This plan includes two additional pages if desired. Those pages can be HTML or text. I will convert straight text to HTML but cannot convert more complex things. I will contact you after receiving this form so that you can e-mail me the text or HTML for those pages.
